Let's Nurture the Neighborhood Together!

Ubuntu LiveWell South Fulton
Health Opportunity Pilot (HOP) Community

Selected as one of six statewide Health Opportunity Pilot (HOP) sites in partnership with the Georgia Health Policy Center.

The six communities in Georgia were chosen for their ambitious 10-year vision to improve access to essential health and well-being conditions. The Health Opportunity Pilot communities will receive financial resources and technical assistance through a Statewide Health Equity Initiative. Funded by the CDC and implemented by the Georgia Department of Public Health and GHPC, this initiative supports innovative approaches to health equity.

This resident-led collaborative will enable problem-solving and solutions development by providing community members with the tools, knowledge, and connections necessary to drive impactful change, particulary in the community-identified areas of behavioral health, early childhood education, environmental justice, housing affordability, youth development, and workforce/economic development.
